All forms are now available on our website. Limited numbers of complete packets will be available in the Bresnahan Main Office (Door #1) after Tuesday, January 19th.
Applications for the 2021-2022 academic year will be accepted beginning
February 1 – February 5, February 8 – February 12, February 22 – February 26: school days only from 10:00AM – 2:00PM at the Bresnahan School Main Office (Door #1). Please note the offices will be closed from February 15 - 19 for school vacation week.
Applications may be turned in at the Bresnahan Office at Door #1,
- or scanned and emailed to lmorasse@newburyport.k12.ma.us or gphillips@newburyport.k12.ma.us,
- or mailed to the school
- or faxed to (978) 465-2112.
A limited number of complete packets will be available at the Main Office (Door #1).
The following forms comprise the Kindergarten Registration Packet:
- Kindergarten Full Day Application OR
- Kindergarten Half Day Application

Kindergarten Parent Questionnaire (please introduce us to your child)
- Preschool Visitation Permission
Please complete the following Newburyport Public Schools District Forms:
- Newburyport District Enrollment Form (English) (Portuguese) (Spanish)
- Confidential Student Health Form
- MA Health Record Form (Doctor Office medical and immunizations records may be substituted)
- Home Language Survey (English version) *
- Certification of Address Form
- Medicaid Consent
* For the Home Language Survey in a language other than English, please contact the office at (978) 465-4435.
In addition you will need to provide:
- Full Day Kindergarten ONLY - $380.00 Registration fee, check or money order made payable to the Newburyport Public Schools (no cash or credit accepted)
Please read the Tuition Policy and Eligibility Criteria Document (keep at home).
Please make copies of each of the following before submitting your application. Be sure to include the copies in your application.
- Your child's birth certificate
- Your child's most recent health record including lead test, immunization record and physical (or MA Health Record Form above)
- Proof of Newburyport Residency (must be one of the following: mortgage/lease agreement or a utility bill [eg: water, electricity, cable, etc.]) Please note residency is defined as where your child sleeps at night; owning property in Newburyport does not automatically make you a resident.
